• come to 340 Appleby Hall to fill out the paperwork electronically and schedule an appointment

or

• download the intake forms
• fill out the information requested
• bring the forms to 340 Appleby Hall and schedule an appointment

Ongoing UCCS clients can call the reception desk at 612-624-3323 to make an appointment with their counselor.
